Back To School Forms

Find, review, sign and submit your school paperwork for the 2022-23 school year.
Required: Parent/Student Handbook Acceptance 
All families/students must acknowledge receiveing and adhering to the SJS Parent/Student Handbook. Sign the Handbook acknowledgement form and return/email to school office. 
Required: AUP (Acceptable Use and Responsibility for Electronic Communication)
All families/students must read the AUP, sign the AUP acknowledgement form and return/email to school office.
Required: Photo/Image Release Form
All families must complete and sign a photo release form for their student(s). Please submit/email to the school office. One form per student. 
Optional: Medication Authorization and Permission Form
This is for parents/guardians of students that may need to have an inhaler or epi-pen left on campus in the health office. Students may not carry any medication on their person or in their backpacks. You do not need to submit a form if your student does not require an inhaler or epi-pen. Please contact the school office directly if you have any questions or concerns. 
Optional: Walk Off Campus Permission Form
This is for parents/guardians of students that are allowed to walk off campus or walk home after school. Student's must have a permission slip on file in the office before they are allowed to walk off campus.
