Look here to find, review, sign and submit your school paperwork for the new school year. Paperwork will be added for families as needed. 
Required: AUP
All families/students must read AUP, sign the AUP acknowledgement form and return to school office. A hard copy of the acknowledgement form was sent home in student family envelope on 8/24/21.
Optional: Medication Authorization and Permission Form
This is for parents/guardians of students that may need to have an inhaler or epi-pen left on campus in the health office. Students may not carry any medication on their person or in their backpacks. You do not need to submit a form if your student does not require an inhaler or epi-pen. Please contact the school office directly if you have any questions or concerns.
